Case Management
Our pregnancy support services provide prenatal education and coordination of care to women experiencing a crisis pregnancy.
Young women are enrolled in a structured Case Management program where there are set standards for personal responsibility and accountability. They work closely with their Case Manager to develop goals that will promote self – sufficiency and are individually coached to learn appropriate life skills. They receive support and education to ensure delivery of a healthy baby and positive parenting.
Adoption information and referrals are available to any woman exploring this decision.
The Pregnancy Support Connection collaborates with the UW Extension, Healthy Families, La Casa de Esperanza, Waukesha Health Department, Parent’s Place and other community resources to provide continuation of care.
All of these services are also available to Spanish speaking women.

Ensuring a safe and healthy baby…
An eighteen year old woman, Lisa, was referred to the Pregnancy Support Connection by a Social Worker from a local hospital because they felt the family needed the additional help and support that PSC is known for in our community. The case manager met with Lisa when she was just twelve weeks pregnant and continued to meet with her monthly throughout her pregnancy. The father of the baby was involved and they had a stable relationship, but neither one of them were employed.
The case manager referred Lisa to WIC, W-2, the food pantry, Tomorrow’s Choice, Goodwill, WIA job training and credit counseling services. The case manager also gave her additional information on breastfeeding, parenting and Born Learning. Lisa did attend childbirth education classes and a budgeting class that PSC provides. She was also referred to a parenting support program and she accepted the program.
She delivered a healthy baby boy that weighed 8.8 lbs and was 21 inches long and was born full term. She plans to attend college in the fall.
